Step 1: Patient Registration
First, we'll ask you to fill out some general details about yourself, including your name, email, and addres.
Step 2: Medical History
Once you've filled out your registration form, you'll be taken to the next page, which is where you'll fill out your medical history.
This will ask you more personalized details about your health history, as well as your current concerns you are seeking help with.
Once you hit submit, we will be sent all of your information!
Step 3: Consent
Finally, you'll be asked to fill out our consent forms, which will allow us to treat your safely and compliantly at Vidal Medical. These will be sent directly to your email!
